    Application fee is $200.00. Dues are $200.00 per year.

    Our application fee at Swedish Hospital is $600 and the Medical Staff Dues are $300

    I'm actually benchmarking as well - our current app fee is $500 for physicians and annual dues are $250 - APPs are assessed half of these amounts.
