   Here is our bylaws excerpt for locums: Locum Tenens: To a practitioner who will be serving as a locum tenens for a current staff member but only after receipt of a complete application for appoint- ment as a locum tenens including a request for specific privileges and at least the following: • Primary source verification of education and training; • Verification of current licensure and DEA; • Current liability insurance • Copy of privilege form from a facility where practitioner currently holds privileges and department chair evaluation; and • Positive written reference specific to the privileges requested from a responsible medical staff authority. Locum tenens is granted initially for 30 days in length, subject to renewal every 30 days, and cannot exceed 120 cumulative days. After that period, the applicant may apply for full membership/privileges.
