3-Year Recredentialing Cycle

For those of you working for healthcare organizations on a 3-year recredentialing cycle, how do you maintain compliance with the CMS CoP requirement that surgical privileges must be reviewed at least every 2 years? We are TJC accredited and looking to move from the 2-year to 3-year cycle.

In Illinois, how long must we retain one's credentialing file? Now that we are electronic, those files stay indefinitely, but I am questioning the retention time for our old paper credentialing files.
